CMS’s preliminary 2027 CLFS rate data confirms significant downward pressure on lab reimbursement — molecular and genomic labs face the largest projected reductions, and the public comment window closes October 21.

Preliminary 2027 Medicare Lab Rates Show Broad Cuts; Comment Window Closes October 21.

On September 21, CMS released preliminary CY 2027 CLFS payment rates based on the second full PAMA private-payor data reporting cycle. Of 1,528 priced codes, 1,171 carry weighted median rates below current 2026 levels — an overall average of approximately 16% lower. Molecular pathology tests are projected down 22% and genomic sequencing down 23%, the deepest cuts of any category. Labs should run code-level revenue impact models now and submit comments to CMS before the October 21 deadline if any preliminary medians appear incorrect.

  • Q4 CLFS/CLIA Update Is Live. CR 14569 took effect October 1, adding new CPT codes, deleting others, and incorporating quarterly CLIA and waived-test updates. New codes carry MAC-specific pricing until CMS establishes national rates. Confirm your billing system has loaded all code changes to prevent rejections on claims dated October 1 and later.

  • ICD-10 FY2027 Codes Now in Effect. The FY2027 ICD-10-CM update, effective October 1, 2026, introduces 190 new codes, 30 deletions, and four revisions, applicable through September 30, 2027. Any claim submitted with a deleted or invalid diagnosis code will reject; verify your LIS and billing platform are updated.

  • Anthem Now Covers Alzheimer’s Blood Biomarker Tests — Documentation Required. Anthem’s revised medical policy, effective October 1, 2026, establishes medical-necessity criteria that could support reimbursement for several blood-based Alzheimer’s assays. Patients must generally be at least 50 years old with measurable cognitive impairment of insidious onset, and the diagnosis must remain unclear after a standard evaluation. Establish a pre-collection documentation checklist to capture required clinical detail and avoid downstream denials.

  • UHC Community Plan: Flow Cytometry and Fecal Calprotectin Policies Incoming. UnitedHealthcare Community Plan will implement a Flow Cytometry Policy reimbursing immunophenotyping codes only when billed for certain specified conditions. Effective November 1, 2026, a new Fecal Calprotectin Testing Policy will similarly limit reimbursement to designated clinical indications. Review Medicaid managed care claims for both test types now and confirm diagnosis linkages meet the new criteria before November 1.

  • NCD Edit Software Updated; Diagnosis Mismatches Will Deny. CR 14536, effective October 1 with MAC implementation October 5, adds and deletes ICD-10-CM codes within the laboratory NCD edit module. Billing staff should cross-check current NCD code lists in the Medicare Coverage Database — a diagnosis not on the approved list is an automatic denial with no gray area.
