Anthem eliminates Modifier 90 pass-through billing effective December 1, 2026. Beginning December 1, 2026, Anthem’s new policy requires that laboratory tests performed by a reference laboratory be billed by the laboratory that performed the service — eliminating reimbursement for pass-through billing using Modifier 90. The policy covers Anthem/Elevance Health’s Medicare Advantage, Commercial, and Medicaid lines in multiple states, including Ohio, Wisconsin, California, Indiana, Kentucky, Missouri, New York, and Colorado, with additional markets expected to follow. Any lab currently collecting, sending to a reference lab, and billing Anthem directly under Modifier 90 must restructure billing workflows before December 1 to avoid denials.
